10/10/2023 EnchantaVaughn Approved (AM)Upon arrival to the school setting, the client appeared to be in a content state as evidenced by greeting QMHP-C with moderate emotions. During morning observations, the client exhibited difficulty with following directions on the first prompt and remaining focused while completing his station work. During midday observations, the client followed directions on the first prompt in the cafeteria for lunch. During afternoon observations, the client exhibited difficulty interacting with his peers in a positive manner during the class reading circle. QMHP-C greeted the client upon arrival to the school setting. QMHP-C inquired about the client’s morning. During morning observations, QMHP-C redirected the client and reiterated the importance of following directions on the first prompt and remaining focused while in his class. During midday observations, QMHP-C provided the client with verbal praise and encouraged him to continue to follow directions on the first prompt. During afternoon observations, QMHP-C redirected the client and reiterated the importance of interacting with his peers in a positive manner. The client responded well towards Therapeutic Day Treatment services throughout the day as evidenced by participation and positive feedback. During morning observations, the client was receptive to redirection and agreed to follow directions on first prompt and to remain focused while completing his class work. During midday observations, the client continued to follow directions on the first prompt while in the cafeteria. During afternoon observations,the client was receptive to redirection and agreed to interact with his peers in a positive manner. The client also agreed to continue to monitor his behavior on his self-monitoring chart.
